About the course 

This face-to-face course is designed to assist Practice Nurses in developing comprehensive chronic disease assessment and management skills. Participants will investigate the creative components of assessment that aim to advance practice to greater proficiency and confidence when working with people with chronic diseases. Participants will also learn about the importance of developing a nurse-led clinic approach in practice to foster an integrative approach to managing patients with chronic disease.

This course is suitable for nurses working within Practice settings.

On completion, participants will be better able to:

• Apply evidence-based policies, models and guidelines that inform chronic disease management in Australia to their own practice 
  
• Demonstrate comprehensive health assessment practices 
  
• Incorporate strategies into their practice that are person-centred and?holistic
  
• Identify the role of the nurse and integrated support services in providing person-centred chronic diseases management??

Biography - Linlin Ho, RN, BN, MN (Nursing Ed), ANI
Nurse Educator – Higher Education

Linlin has worked in the primary health care settings since the beginning of her career. She has achieved multiple qualifications that would benefit her as a nurse in primary health care setting, such as Immunisation for Health Practitioners, Well Women’s screening, Cannulation for nurses and Wound closure and Suturing.

Linlin has a passion for community and primary health care nursing education. She believes community healthcare is the fundamental component of our medical system and primary healthcare is the client’s first encounter with our health sectors.
